Tis’ the Season!!! 3D Girls, Inc. is excited to share another year of giving this holiday season. Help us be a blessing to others by nominating a family to receive holiday blessing. Families in need selected to participate will receive toys and support from the partners and affiliates of 3D Girls, Inc.
Please note all nominee’s MUST be able to provide the following documentation to receive holiday gifts:
Valid Photo ID (Driver’s License, Passport)
1.) Proof of Current Address (the document must have the name of the parent/guardian on it)
-A copy of current lease
-Current utility bill
2.) 4.Proof of Income – ALL DOCUMENTS MUST BE CURRENT
-CURRENT Medicaid, TANF, and/or SNAP Benefit Award Letters listing each child as a beneficiary OR
-Free/Reduced school lunch verification OR
-2019 Tax return
3.) Birth Certificate for each child to verify ages
4.) Official custody or guardianship papers if applicant is not the biological parent
Deadlines to complete the nomination process will be Friday, November 30, 2020 at 4:00 PM. Submitted applications DO NOT GUARANTEE SELECTION! However,we will do our absolute best to fulfill requests of items for each family.
Nominations will be reviewed by our team. Selected participants will be notified no later than December 6, 2020.
